Welcome to Katy’s Cat Cafe! We are delighted to have you here to enjoy the company of our friendly feline residents. To ensure the safety and enjoyment of all our guests and the well-being of our cats, we kindly ask you to read and sign this Liability Waiver and Release Form.

1. Acknowledgment of Risk
I, the undersigned, acknowledge that my participation in activities at Katy’s Cat Cafe involves potential risks, including but not limited to scratches, bites, allergic reactions, and other injuries that may result from interacting with cats. I fully understand and accept these risks.

3. Acknowledgement
Katy’s Cat Cafe has available cats to be played with by customers at Katy’s Cat Cafe and agrees to allow me to play with same in consideration of both the payment of the fees for same and my execution of this Release and agreeing to be bound by its terms.

2. Waiver of Liability
I hereby waive, release, and discharge Katy’s Cat Cafe, its owners, employees, volunteers, and agents from any and all liability, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with my participation in activities at Katy’s Cat Cafe, including those allegedly attributed to the negligent acts or omissions of the above parties.

3. Indemnification
I agree to indemnify and hold harmless Katy’s Cat Cafe, its owners, employees, volunteers, and agents from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney’s fees, arising from or connected to my participation in activities at Katy’s Cat Cafe.

4. Medical Treatment
I consent to receive medical treatment deemed necessary if an injury occurs while at Katy’s Cat Cafe, and I agree that I am financially responsible for any costs incurred as a result of such treatment.

5. Compliance with Rules
I agree to comply with all rules and instructions provided by Katy’s Cat Cafe staff to ensure the safety and well-being of both guests and cats.

6. Photography and videography Release
By signing below, I grant Katy’s Cat Cafe permission to use photographs and videos of me taken during my visit for promotional purposes, including social media, with full knowledge and consent. If the individual is under the age of 18, I certify that I am the parent or legal guardian of the individual and have the legal authority to execute this release on behalf of the minor.

7. Membership Clause
If I am a member of Katy’s Cat Cafe, I agree that this Liability Waiver and Release Form applies to all visits and activities at the cafe for the duration of my membership. I understand that I am waiving certain legal rights for the entire lifetime of my membership.

8. Legal Age and Competency
I affirm that I am of legal age and mentally competent to sign this Waiver and Release Form. If signing on behalf of a minor, I affirm that I am the parent or legal guardian and have the authority to grant this waiver.

BY EXECUTING THIS RELEASE, I, ON BEHALF OF MINOR, ACKNOWLEDGE THAT I HAVE READ THIS RELEASE, UNDERSTAND THE CONTENTS HEREOF, HAVE BEEN ADVISED AND HAD THE OPPORTUNITY TO SEEK INDEPENDENT COUNSEL OF MY CHOICE AND CERTIFY THAT I, ON BEHALF OF MINOR, HAVE FREELY AND VOLUNTARILY EXECUTED THIS RELEASE.

9. Severability
If any provision of this waiver is found to be unenforceable or invalid, the remaining provisions shall remain in full force and effect.
