cityWell brooklyn has put in place preventative measures to reduce the spread of COVID-19, such as the wearing of masks, regular and thorough sanitizing, social distancing, etc. However, cityWell brooklyn cannot guarantee that you will not become infected with COVID-19.

By signing this agreement, you acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that you may be exposed to or infected by COVID-19 while using cityWell brooklyn facilities, and that such exposure or infection may result in personal injury, illness, permanent disability, and death.

I understand that I will use cityWell brooklyn facilities at my own risk and will not hold cityWell brooklyn or cityWell staff liable for any accident or injury that may occur.

I understand that I should not use cityWell brooklyn facilities while intoxicated and I assume all liability/responsibilities when consuming alcohol.

I understand that if I have heart conditions, high or low blood pressure, or have health conditions that can be affected by hydrotherapy, or are pregnant I should consult a physician before using the hydro tub, steam room or sauna.
