I, being the responsible party for the above pet, have the authority to grant Dog Spaw my consent to receive, bathe, and groom, etc., my pet. I understand that grooming can be stressful for some animals and especially the geriatric. I also understand that it may be necessary for my pet to be left unfinished if the grooming is not being tolerated by the pet, partial grooming charges accrued. It is also understood that Dog Spaw will use all precautions against the loss, sickness or death of your pet. We will not be held responsible for any pre-existing conditions that was not told to the groomer beforehand or for animals that have not been under the regular care of a veterinarian. If a pre-existing condition is made known to the groomer it will be left up to the groomer to accept the animal for grooming.
I will not hold Dog Spaw responsible for any unexpected complications, I understand that I will assume all risk and costs due to any abnormal findings or emergency care. I understand that my pet may not be used to being groomed and that there are inherent risks in grooming. If an emergency occurs I give permission for the Dog Spaw to obtain emergency medical care and that I will be notified as soon as possible in the advent of said emergency and will be responsible to the veterinarian for any medical costs accrued. In the event of an emergency, I authorize Dog Spaw to take my pet to the nearest Veterinary Clinic for treatment.
I am consent that photos and/or videos of my pet can be used by Dog Spaw 2 Hour Grooms on Social Media for advertising and promoting purposes.
I am responsible to have my pet picked up. I understand that a late pickup fee could be added if my pet is left for more than the usual time.

Please give 24 hours advance notice in the event of cancellation. After 2 cancellations or "No Shows", customers will have to prepay for their next appointment.

There is a $65 per hour charge for dematting dogs, billed in 5 minute increments.

1. For any dogs that are matted, we will take every precaution to safely remove them, including shaving if necessary. WE ARE NOT RESPONSIBLE FOR ANY NICKS OR CLIPPER IRRITATION AS A RESULT OF SEVERE MATTING.
2. Please make us aware of any skin conditions prior to be groomed.
3. For dogs that are severely matted, we will not brush them out, we will shave them and there will be an additional blade charge.

By signing this form you agree to hold Dog Spaw 2 Hour Grooms harmless.
